The ECFMG accepts two English tests: TOEFL iBT (Test of English as a Foreign Language, internet-based) and IELTS Academic. TOEFL Speaking: Microphone vs Examiner

The most significant difference is Speaking. TOEFL Speaking requires you to speak into a microphone in a room full of other test-takers — there is no human examiner. You listen to audio prompts and respond within strict time limits (15–45 seconds preparation, 45–60 seconds response). This format favours candidates who are comfortable with timed responses and do not need the visual cues of a human conversation. IELTS Speaking, by contrast, is a face-to-face interview with a human examiner lasting 11–14 minutes — more natural but also more unpredictable.

TOEFL Integrated Writing: A Unique Challenge

TOEFL's Integrated Writing task has no IELTS equivalent. You read a passage, listen to a lecture that contradicts or supports it, then write a summary showing how the two relate. This tests a combination of reading, listening, and writing skills simultaneously — a format closer to what you will experience in US clinical rotations where you synthesise information from multiple sources. IELTS Writing Task 1 (graph description) and Task 2 (essay) are tested independently.

USMLE Pathway from India — Complete Roadmap

The United States Medical Licensing Examination (USMLE) is a three-step exam that all doctors must pass to practise medicine in the US. For Indian IMGs, the pathway is longer and more expensive than the UK's PLAB route, but the rewards — in terms of salary, specialisation, and career growth — are among the highest in the world.

Step 1: USMLE Step 1 (Pass/Fail)

Step 1 tests foundational medical sciences (anatomy, physiology, pharmacology, pathology, microbiology, biochemistry). Since January 2022, it is scored as Pass/Fail (previously a 3-digit score). You can take Step 1 at Prometric centres in India. The fee is $645 (₹54,000). Most Indian doctors prepare for 3–6 months using resources like First Aid, Pathoma, and UWorld.

Step 2 CK: Clinical Knowledge (Scored)

Step 2 CK is now the most important scored exam for residency matching. It tests clinical medicine across all specialties. A score of 240+ is competitive for most specialties, and 250+ for competitive specialties (dermatology, orthopaedics, ophthalmology). Fee: $645. Preparation: 3–6 months. This is the exam that residency programmes weigh most heavily.

ECFMG Certification

After passing Step 1 and Step 2 CK, and meeting the English proficiency requirement (TOEFL or IELTS), you apply for ECFMG Certification. This also requires verification of your medical school credentials. The ECFMG certificate is your ticket to apply for US residency programmes through the NRMP Match.

Residency Match (NRMP)

The National Resident Matching Program (NRMP) is the annual process through which medical graduates are matched to residency training positions. Indian IMGs typically apply to 100–300 programmes (cost: $10–25 per application). Interview season runs October–February. Match Day is in March. The Match rate for IMGs varies by specialty: Internal Medicine (~55%), Family Medicine (~50%), Psychiatry (~45%), Pathology (~60%).

Step 3: After Starting Residency

Step 3 is taken during residency (usually PGY-1 year). It tests independent practice readiness. Fee: $895. After completing residency (3–7 years depending on specialty), you can obtain a US medical licence and practise independently. Specialist physician salaries range from $250,000 to $600,000+ per year.
