TOEFL for USMLE is the English proficiency pathway that every Indian doctor must clear to pursue a medical career in the United States. The Educational Commission for Foreign Medical Graduates (ECFMG) requires proof of English proficiency as part of its certification process, and TOEFL for USMLE remains the primary choice for US-bound international medical graduates (IMGs). While IELTS Academic is also accepted, TOEFL iBT is the test most aligned with the American academic and clinical context. In this guide, EEC's medical career team explains the ECFMG requirement, the complete USMLE pathway from India, TOEFL vs IELTS for US medical careers, and how EEC coaches both tests at ₹7,500 to help Indian doctors reach their American dream.

The ECFMG (Educational Commission for Foreign Medical Graduates) is the gatekeeper for all international medical graduates seeking to practise medicine in the United States. Before you can sit for the USMLE Step exams or enter a US residency programme, you must obtain ECFMG Certification. One of the requirements for this certification is demonstrating English language proficiency through an approved test.
The ECFMG accepts two English tests: TOEFL iBT (Test of English as a Foreign Language, internet-based) and IELTS Academic. There is no single mandated minimum score — the ECFMG evaluates English proficiency as part of a holistic assessment. However, most residency programmes and medical schools expect TOEFL iBT scores of 79–100+ or IELTS 7.0–7.5. Competitive programmes at top university hospitals often look for TOEFL 100+ or IELTS 7.5+. The score is valid for 2 years from the test date.

TOEFL Speaking: Microphone vs Examiner
The most significant difference is Speaking. TOEFL Speaking requires you to speak into a microphone in a room full of other test-takers — there is no human examiner. You listen to audio prompts and respond within strict time limits (15–45 seconds preparation, 45–60 seconds response). This format favours candidates who are comfortable with timed responses and do not need the visual cues of a human conversation. IELTS Speaking, by contrast, is a face-to-face interview with a human examiner lasting 11–14 minutes — more natural but also more unpredictable.
TOEFL Integrated Writing: A Unique Challenge
TOEFL's Integrated Writing task has no IELTS equivalent. You read a passage, listen to a lecture that contradicts or supports it, then write a summary showing how the two relate. This tests a combination of reading, listening, and writing skills simultaneously — a format closer to what you will experience in US clinical rotations where you synthesise information from multiple sources. IELTS Writing Task 1 (graph description) and Task 2 (essay) are tested independently.

USMLE Pathway from India — Complete Roadmap
The United States Medical Licensing Examination (USMLE) is a three-step exam that all doctors must pass to practise medicine in the US. For Indian IMGs, the pathway is longer and more expensive than the UK's PLAB route, but the rewards — in terms of salary, specialisation, and career growth — are among the highest in the world.
Step 1: USMLE Step 1 (Pass/Fail)
Step 1 tests foundational medical sciences (anatomy, physiology, pharmacology, pathology, microbiology, biochemistry). Since January 2022, it is scored as Pass/Fail (previously a 3-digit score). You can take Step 1 at Prometric centres in India. The fee is $645 (₹54,000). Most Indian doctors prepare for 3–6 months using resources like First Aid, Pathoma, and UWorld.
Step 2 CK: Clinical Knowledge (Scored)
Step 2 CK is now the most important scored exam for residency matching. It tests clinical medicine across all specialties. A score of 240+ is competitive for most specialties, and 250+ for competitive specialties (dermatology, orthopaedics, ophthalmology). Fee: $645. Preparation: 3–6 months. This is the exam that residency programmes weigh most heavily.
ECFMG Certification
After passing Step 1 and Step 2 CK, and meeting the English proficiency requirement (TOEFL or IELTS), you apply for ECFMG Certification. This also requires verification of your medical school credentials. The ECFMG certificate is your ticket to apply for US residency programmes through the NRMP Match.
Residency Match (NRMP)
The National Resident Matching Program (NRMP) is the annual process through which medical graduates are matched to residency training positions. Indian IMGs typically apply to 100–300 programmes (cost: $10–25 per application). Interview season runs October–February. Match Day is in March. The Match rate for IMGs varies by specialty: Internal Medicine (~55%), Family Medicine (~50%), Psychiatry (~45%), Pathology (~60%).
Step 3: After Starting Residency
Step 3 is taken during residency (usually PGY-1 year). It tests independent practice readiness. Fee: $895. After completing residency (3–7 years depending on specialty), you can obtain a US medical licence and practise independently. Specialist physician salaries range from $250,000 to $600,000+ per year.

Which Test to Choose — TOEFL or IELTS for US Medicine?
The decision between TOEFL and IELTS for your ECFMG certification depends on your career goals, test-taking preferences, and backup plans:
Choose TOEFL If:
You are targeting ONLY the US. You prefer speaking into a microphone over a face-to-face interview. You type fast and are comfortable with 100% computer-based testing. You want the Integrated Writing task that mirrors US academic expectations. Most US residency programme directors are more familiar with TOEFL scores.
Choose IELTS If:
You want UK (PLAB/GMC) or Australia (AMC/AHPRA) as backup options. You prefer face-to-face Speaking with a human examiner. You want CD-IELTS with OSR (One Skill Retake) as a safety net. IELTS is accepted by ECFMG, GMC, NMC, and AHPRA — the most versatile single test for doctors.
Choose Both If:
You want to maximise your chances and submit whichever score is higher. Budget allows ₹33,000–35,000 for both tests. You are applying to programmes in multiple countries. Taking both gives you flexibility and a backup plan.
“I took both TOEFL and IELTS on EEC's advice. Scored TOEFL 108 and IELTS 8.0. Used the TOEFL score for my ECFMG application and the IELTS score as a backup for UK GMC. Matched into Internal Medicine at a university hospital in Chicago. EEC's dual-test coaching saved me from putting all my eggs in one basket.”

TOEFL Score Strategy — Targeting 100+ for Competitive Programmes
TOEFL iBT is scored 0–120, with each section (Reading, Listening, Speaking, Writing) scored 0–30. For competitive US residency programmes, target a total score of 100+ with no section below 22. Here is the section-by-section strategy:
← Swipe left to see more columns →
| Section | Score Range | Target | Key Strategy |
|---|---|---|---|
| Reading | 0–30 | 26–28 | Academic passages, inference questions, vocabulary in context |
| Listening | 0–30 | 26–28 | Note-taking during lectures, understanding implied meaning |
| Speaking | 0–30 | 24–26 | Timed responses, integrated tasks, clear pronunciation |
| Writing | 0–30 | 25–27 | Integrated task (read+listen+write), independent essay with examples |
| Total | 0–120 | 100–110 | 8–12 weeks preparation with structured practice |
Reading: TOEFL Reading uses academic passages similar to textbook chapters. Practice with academic articles from Scientific American, Nature, and university textbook excerpts. Focus on inference questions (what the author implies) and vocabulary-in-context questions. Time management: 17–18 minutes per passage.
Listening: TOEFL Listening features university lectures and campus conversations. Take notes using abbreviations and symbols. Focus on understanding the speaker's attitude and purpose, not just factual content. Practice with TED-Ed lectures and TOEFL-specific materials.
Speaking: The four Speaking tasks include independent opinion questions and integrated tasks where you read, listen, and then speak. Practice recording yourself with 15–30 second preparation and 45–60 second responses. Clarity and organisation matter more than accent. Use the template: state opinion → give reason → provide example → conclude.
Writing: The Integrated Writing task requires you to summarise how a lecture challenges or supports a reading passage. Use the template: introduction (state relationship) → paragraph 1 (first point of contrast) → paragraph 2 (second point) → paragraph 3 (third point) → conclusion. Target 280–320 words. The independent essay (300+ words) should have a clear thesis, 2–3 body paragraphs with examples, and a conclusion.
